How Client Directed Home Care Works

1. Get Approved

Your eligibility for CDHCI and the home support services available to you are determined through your home care assessment and approved care plan.

2. Choose Your Provider

Once approved, you can choose the registered CDHCI provider you would like to deliver your authorized home support services.

3. Arrange Your Care

Work with TrustBloom to arrange your approved services and a care schedule that works for you. You remain involved in the decisions about who provides your care and how your approved services are scheduled. We take care of the eligible direct-billing process.
